Your inital appointment with South Jersey Periodontics involves a consultation to address any problem or concern found by you or your general dentist. Dr. Shah will complete an in-depth medical and dental health history as well as a thorough examination to address your concern.
Please assist us by providing the following information at your first consult:
- Your referral form and any x-rays from your referring dentist.
- A list of medications you are currently taking.

Any dental insurance information.

You can complete the New Patient Forms online. Alternatively, please arrive 15 minutes early to your first appointment to allow ample time to complete these forms.
Will It Hurt?
Dr. Shah is always as gentle as possible. The periodontal exam can be completed with little or no discomfort.
Do I Need X-Rays?
We will need current periodontal x-rays in order to see disease not otherwise visible. If your referring dentist has taken x-rays, you may request that they be forwarded to us. Please make arrangements with your general dentist for these x-rays to be forwarded before your initial exam. A 3D x-ray may be needed for quality diagnostics for before certain treatments.

Will I Need Surgery?
Not everyone needs periodontal surgery. If treated early, gum disease can be controlled without surgery. We will make recommendations based on your individual situation. Our philosophy of practice is to treat as conservatively as possible to attain treatment goals.
